Background and Aim: The "feminization of migration" has assumed new dimensions in recent years, particularly among healthcare professionals. In Iran, this phenomenon holds heightened significance given the structural, professional, and gender-based inequalities within the healthcare system. This study systematically identifies and analyzes the factors shaping the migration intentions and actions of female healthcare workers based on existing empirical evidence.
Data and Method: A systematic review was conducted by searching national (2005–2025) and international (2015–2025) scientific databases. Following rigorous screening against predefined inclusion and exclusion criteria, 25 eligible studies were selected. Extracted data were synthesized and interpreted using thematic analysis, anchored in push–pull, human capital, and globalization theoretical frameworks.

Findings: The findings reveal that female healthcare workers' migration stems from the interaction between domestic push factors—including low remuneration, restricted career advancement opportunities, heavy workloads, gender discrimination, and institutional instability—and foreign pull factors, such as superior working conditions, job security, higher quality of life, and gender equality in destination countries. Additionally, age, educational attainment, and prior migration experience functioned as moderating variables in migration decision-making.

Conclusion: Based on the synthesized model, the migration of female healthcare workers in Iran cannot be reduced to a mere economic reaction. Rather, it reflects the structural and gendered limitations of the health system in offering sustainable professional and social horizons for female specialists. Within this context, migration represents a strategic mechanism to reclaim professional and social agency, rather than simply a loss of human capital.

Key Message: The migration of female healthcare workers reflects shifting professional and gender expectations among Iranian female specialists—a process whose ramifications extend beyond human capital flight to directly impact health system sustainability and social policy-making.
