تجربه زیسته زنان سالمند از سواد سلامت در شهر اصفهان: درهم‌تنیدگی مراقبت‌های درمانی، مرجعیت خانواده و بازماندگی از جهان دیجیتال

چکیده

زمینه و هدف:  با افزایش جمعیت سالمندان و شیوع بیماری‌های مزمن، سواد سلامت به یکی از مؤلفه‌های کلیدی سلامت عمومی تبدیل شده است. با این‌حال، درک تجربه زیسته زنان سالمند از سواد سلامت، به‌ویژه در بافت فرهنگی و اجتماعی ایران، کمتر مورد توجه قرار گرفته است. هدف این پژوهش، فهم و تفسیر تجربه زیسته زنان سالمند شهر اصفهان از مفهوم و کاربرد سواد سلامت است.
 
روش و دادهها: این مطالعه با رویکرد کیفی و روش پدیدارشناسی تفسیری انجام شد. مشارکت‌کنندگان شامل ۱۴ زن سالمند ۶۰ سال و بالاتر ساکن شهر اصفهان بودند که به روش نمونه‌گیری هدفمند انتخاب شدند. داده‌ها از طریق مصاحبه‌های نیمه‌ساختاریافته عمیق گردآوری و براساس اصول پدیدارشناسی تفسیری تحلیل شدند.
 
یافتهها: تحلیل داده‌ها به استخراج مضامین تفسیری منجر شد که نشان‌دهنده تجربه‌ای پیچیده، بافت‌مند و اجتماعی از سواد سلامت بود. مضامین اصلی شامل گم‌شدن در هیاهوی توصیه‌های متناقض، خانواده به‌مثابه مرجع نهایی سلامت، شنیده‌نشدن بدن سالمند در نظام درمان، خستگی از درمان ناتمام، نادیده‌ماندن آموزش سالمندان، عقب‌ماندن از سلامت دیجیتال، سلامت به‌مثابه توان زیستن و سلامت در سایه تنگناهای اجتماعی- اقتصادی بود.
 
بحث و نتیجهگیری: یافته‌ها نشان داد که سواد سلامت در میان زنان سالمند نه صرفاً یک مهارت شناختی، بلکه تجربه‌ای زیسته و معنا‌مند است که با ایمان، روابط اجتماعی، اعتماد و شرایط ساختاری گره خورده است.
 
پیام اصلی: سواد سلامت زنان سالمند در تعامل با خانواده، نظام سلامت و شرایط اجتماعی شکل می‌گیرد و نمی‌توان آن را صرفاً یک توانایی فردی دانست. یافته‌های پژوهش نشان می‌دهد که هم‌زمانی تداوم برخی الگوهای فرهنگی با تغییرات اجتماعی و دیجیتالی، تجربه سالمندی را دگرگون کرده است؛ ازاین‌رو، برنامه‌های ارتقای سواد سلامت باید متناسب با این تحولات و با تکیه بر تجربه زیسته سالمندان طراحی شوند.

Background and Aim: Along with population aging and the rising prevalence of chronic diseases, health literacy has emerged as a critical public health concern. Nevertheless, older women’s lived experiences of health literacy within specific socio-cultural contexts remain under-researched. This study aimed to explore and interpret the lived experiences of older women in Isfahan regarding how health literacy is conceptualized, experienced, and enacted in daily life.
 
Data and Method: Utilizing an interpretive phenomenological approach, this qualitative study investigated the lived experiences of older adults. Participants comprised fourteen women aged 60 and older residing in Isfahan, selected through purposive sampling. Data were collected using in-depth, semi-structured interviews and analyzed in accordance with interpretative phenomenological analysis (IPA) principles.
 
Findings: Interpretive analysis revealed several themes reflecting a complex, contextual, and socially embedded experience of health literacy. Core themes included: navigating conflicting health advice, family as the primary health authority, the unheard aging body within healthcare encounters, exhaustion from fragmented care, educational marginalization of older adults, digital health disparities, health as functional independence, and health negotiating structural and economic constraints.
 
Conclusion: Health literacy among older women is not merely a cognitive capacity, but a relational, lived experience deeply intertwined with faith, social networks, interpersonal trust, and broader structural conditions.
 
Key Message: Health literacy among older women is a relational construct negotiated through interactions with family, the healthcare system, and broader socio-economic conditions; hence, it cannot be reduced to an individual cognitive skill. The findings highlight that the convergence of enduring cultural norms with rapid social and digital transformations fundamentally reshapes the experience of aging. Consequently, health literacy interventions must move beyond functional knowledge to address these social shifts while remaining firmly grounded in the lived realities of older adults.
